Description
Customer service is the name of the game and as an Admissions Coordinator you will play a pivotal role in the day to day office operations.
You will triage the incoming calls from patients and families, initiate the referral process, verify insurance, keep the Executive Director informed on the day to day activities, and so much more.

Qualifications
On an Average Day You Will :

Promote an environment which facilitates quality care, customer service and a spirit of cooperation among home care team membersAct as a liaison between private, HMO or federally funded insurance companies and the agencyTriage incoming calls related to patients, families, physicians and staffInitiate referral process as outlined in department specific procedureAct as a liaison for hospital case managers providing resource information options tohelp coordinate services as neededAccept information for patient referrals for agency services from medical office staff, physicians, hospital discharge planners and private individualsVerify MD licensureKeep ED informed of day-to-day activities, insurance case issues and patient care concernsWork with ED to keep agency in compliance with all regulatory, licensing and accreditation requirementsActively participate in performance improvement activities as directed by EDCollect data from clinical records and field staff, for insurance verification to staff treatment plans and services needed by patientsKeep ED informed of day-to-day activities, personal accomplishments, staff concernsand problemsAssist with new hire orientationCo-facilitate IDT with ED as needed to ensure flow of process and documentationMonitor census and track homecare admissions, discharges, certification and re- certification dates on all patients and provide this information for IDT to staff providing services to patientsCoordinate OASIS transmission to the state meeting regulatory guidelinesParticipate in and assists with community activities, i.e. health fairs, conventions, community education programs, etc. as assigned by the Executive DirectorAll other duties as assignedRequirements :
Must have one (1) year hospice experienceMust have two (2) years of admissions experienceMust be 18 years oldPR certification preferredWe Got the Perks :
