• Experience in assessing patients, care coordination, and discharge planning.
• Ability to organize and participate in family meetings.
• Experience with treatment plan development.
• Knowledge of adoption / foster care, drug / alcohol treatment options, and mental health crisis management.
• Competence in arranging durable medical equipment, skilled nursing facilities, and ambulance transport.
