Key Responsibilities

• Lead and Mentor : Supervise and develop PCPs to guarantee high-quality, value-based care.
• Deliver Care : Provide direct patient care as needed, while balancing clinical and leadership responsibilities.
• Team Leadership : Oversee interdisciplinary care teams, including medical assistants, care coordinators, and facilitators.
• Care Coordination : Work collaboratively with hospitalists, specialists, and transitional care teams for seamless patient experiences.
• Operational Excellence : Partner with Center Operations and Market Leadership to enhance clinical performance and financial outcomes.
• Culture Building : Foster a positive, high-performing clinical culture through regular meetings and leadership engagement.
• Center Support : Provide clinical leadership support as needed, monitor daily census, and assist in provider and patient recruitment.
