Job Description

DUTIES

  • Coordinates, monitors, develops and implements projects and / or initiatives for CIB / Chronic Disease Prevention and Control.
  • Coordinates and monitors program activities to ensure effective operations and compliance with established standards and / or contracted goals & objectives.
  • Reviews, enters, and analyzes information and data concerning BPHC program, to determine effectiveness; makes recommendations for changes in procedures; provides technical assistance and advice to staff and others; responds to inquiries and maintains liaison with various external agencies to provide information concerning program.
  • Collaborates with various CBOs to exchange information and / or resolve problems. As a member of a multidisciplinary team, provides health education to clients, in homes, schools and community based settings. Represents BPHC and program at meetings, health fairs and conferences.
  • Coordinates activities which may include : fiscal monitoring; grant writing; monitoring of funded programs or agencies to ensure compliance; report preparation and writing; and assisting with developing and distributing communications, brochures and educational materials. May supervise employees.
  • Working to help BPHC become an antiracist organization by meeting or exceeding the standards set by the BPHC Anti-Racism Policy.
  • Performs other duties as required.
