Summary The Marion VA Healthcare System is seeking a full-time Registered Nurse to serve in the role as Case Manager for the Home Based Primary Care Team. located in Vincennes, IN Community Based Outpatient Clinic. The Registered Nurse in this position provides health care services and care management to high risk patients in their homes. Responsibilities The Care Manager is accountable for meeting and maintaining the qualification standards and functional statements of assigned grade. The incumbent must posses good organizational skills which enable completion of work, demonstrated ability to assume leadership in a patient centered program and the ability to establish effective communication with patients, families, visitors and members of the interdisciplinary team. The incumbent assesses, plans, implements and evaluates patient care needs assuring quality of care. Not all duties may be listed, so other duties may be assigned : not limited to : Case Management. Patient Assessments. Participation in interdisciplinary team process. Home care visits. Performing skilled nursing tasks (i.e., blood draws, injections, dressing changes and wound care). Completes and submits clinical notes. Observes patient for changes in conditions and reports appropriately. Provides education instructions to patients and their designated significant other. This position requires that the employee operate a government motor vehicle. Performs other professional nursing duties as assigned and within their scope of practice. Other duties as assigned. This position potentially requires flexibility in schedule and assignments. PLEASE NOTE : These positions require Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications. If selected, Applicants will provide proof of current certifications prior to employment. Only BLS certification which included Automated External Defibrillator (AED) and ACLS certification obtained from either the American Heart Association (AHA) or the Military Treatment Training Network (MTN) will be accepted. Qualifications Basic Requirements : English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant : The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree / entry level Nursing education program. Credit for foreign nursing education higher that associate degree / entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception : Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.) NOTE : Grandfathering Provision - All persons currently employed in VHA in 0610 series and performing the duties as described in the qualification standard on the effective date of the standard (1 / 29 / 2024) are considered to have met all qualification requirements for the grade held including positive education and licensure / certification. Nursing bridge programs are accelerated nurse education programs which rely upon existing nursing knowledge and education. Graduates from nursing bridge programs that meet accreditation requirements specific grade level requirements in this standard must also be met in addition to the following : (a) Master's Level Bridge Program Education Appointments Prior to Graduation. Individuals attending a master's level bridge program in nursing who achieve the level in their education in which they have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on a temporary basis for any period up to three years contingent upon full completion of the professional nursing education program, if education verification provided by the school supports the Bachelor of Science in Nursing (BSN) equivalency. Upon successful completion and graduation from the bridge program, the Nurse may be converted to a permanent appointment under the provisions of section 7401(a)(1) of title 38. Failure to complete the bridge program on which the temporary appointment was based will result in termination of the appointment. (3) Foreign Education. Graduates of foreign schools of professional nursing who possess a current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing to warrant appointment as a Nurse who has completed an associate degree level Nursing education program. In order to grant credit towards an appointment based on foreign nursing education higher than the associate degree level, formal degree equivalency validation from a recognized degree equivalency evaluation agency accepted by VA such as the International Consultants of Delaware (ICD)must be obtained.
