Program Palliative Care Manager

Summary & Objective

The Manager Program Palliative Care is responsible for directing and coordinating the overall functions that create a concise clear path for patients to move on the continuum of care from palliative to hospice or other CHS program.

Essential Functions

• Ensure effective communication with Marketing Field Admissions Team Referral Sources and Hospice Interdisciplinary team for assigned facility.
• Serves as the program expert and go to person for hospice physicians hospice clinical staff and assigned hospice admissions staff including marketing team members.
• Coordinates the daily operations between hospice and palliative teams to contribute to smooth admission process.
• Takes proactive steps to minimize delays in delivery of services.
• Regularly communicates with customers / clients to arrange meetings services and discuss any hospice or palliative issues.
• Regularly communicates with management regarding the status to effectively move patients through the continuum of care.
• Identifies staffing needs proactively and communicates with management team.
