Clinic Care Guide

Job Description

: The Clinic Care Guide serves as a healthcare navigator for identified patients and their caregivers. This role is responsible for serving as a single point of contact for patients, providing outreach and supporting patients with education around preventative care and chronic disease management, referral management and transitions of care. The embedded / centralized care guide leads the daily team huddle and engages in pre-visit, visit and post visit planning to support the health care team in care gap closure, care navigation and care plan outcome

attainment to efficiently deliver proactive, patient-centered care. This position works collaboratively with the care management team, patients, caregivers, healthcare providers, payers, community-based providers, and other involved parties to provide effective, efficient, value-based care services.
