Established to ensure that incarcerated individuals receive care that meets constitutional standards, CCHCS provides a range of services including preventative care, chronic disease management, emergency services, and substance use disorder treatment. Primary Care Physician - RESPONSIBILITIES (listed are included but not limited to)
- The Primary Care Physician diagnoses medical conditions and order appropriate laboratory tests, imaging studies (e.g., X-rays), and other diagnostic procedures as needed.
- Collaborate with other agencies or health facilities on behalf of patients and their families to arrange admissions to hospitals, rehabilitation facilities, and skilled nursing facilities.
- Organize emergency ground and / or air medical transport as required.
- Analyze clinical data, develop therapeutic programs, and communicate information effectively to patients and their families.
- The Primary Care Physician provides support to patients and families in times of illness.
- Maintain accurate and timely Electronic Health Records (EHR) with detailed clinical documentation, including diagnosis codes. Complete all EHR notes within 24 hours of patient encounters.
- Prescribe medications and treatment therapies in coordination with nurses, pharmacists, and other healthcare personnel.
- Follow up on treatment plans and monitor patient progress.
- Recognize the need for referral to a mental health professional when necessary.
- The Primary Care Physician consults independently with medical and surgical specialists to discuss patient care and arrange referrals as needed to appropriate health facilities.
