University Nursing & Rehabilitation Center -

University Nursing and Rehab is seeking a caring and passionate Part-time Social Worker to join our Social Services Team.

Responsibilites :

  • Identifying individual social and emotional needs
  • Developing and Maintaining individualized care plans
  • Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
  • Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

Part : (Monday, Wednesday, Friday 8 : 30am-5 : 00pm)

Requirements :

  • Previous experience working in long-term care is preferred
  • Bachelors degree in Social Work, Sociology, Psychology, or Human Services is Preferred.
