This opportunity is located at our Diabetes and Internal Medicine Care at 8205 East 56th Street, Suite 100 Indianapolis, IN 46216.

Highlights

Community Health Network has an excellent opportunity for a family medicine or internal medicine trained Advanced Practice Provider to work at our outpatient Internal Medicine Care, North practice.

• Patients aged 18 years and older
• A comprehensive and integrated electronic medical record system (EPIC)
