Occupational Therapist
Work with older adults in a community-based care setting where the focus is on helping participants stay safe, independent, and active in their own homes. This Occupational Therapist position is part of a local PACE (Program of All-Inclusive Care for the Elderly) team, providing in-center and in-home support, evaluations, and collaborative care planning. You’ll play a key role in helping seniors maximize their function and quality of life while working in a supportive clinical environment.
What You’ll Do :
- Complete initial evaluations and reassessments for PACE participants
- Develop and implement individualized treatment plans
- Provide home safety evaluations and ADL-focused recommendations
- Educate participants, caregivers, and staff on therapy plans and techniques
- Collaborate with Home Health RNs and PCW Supervisors
- Assist in discharge planning from hospitals and skilled nursing facilities
- Recommend and coordinate adaptive equipment and DME
- Document care in the electronic medical record (EMR)
- Participate in interdisciplinary team meetings and quality improvement efforts
- Support clinical competency training for aides as needed
What You’ll Need :
Active Occupational Therapist licenseCurrent CPR certification2 to 3 years of therapy experience preferredExperience with frail elderly or chronically ill individuals strongly preferredComfortable working with high-needs populations and in a team settingStrong communication, organization, and adaptabilityReliable personal vehicle for local travelBilingual skills a plus (but not required)Ability to perform physical job duties with or without accommodationsWhy You Should Apply :
