Work Hours : Monday-Friday, 8 : 00 AM - 8 : 00 PM CST (rotating shifts; occasional extended hours or weekends)

Rate : $60-62 / hr

About the Role

The Clinical Engagement Services Center provides a variety of clinical services including adherence, diabetes management, fraud waste and abuse prevention, medication therapy management (MTM), retrospective drug utilization review, and STARS adherence.

As a Clinical Pharmacist, you will perform telephonic consultations with members to complete comprehensive medication reviews, improve disease state management, and provide patient education. You will document care delivered, communicate with primary care providers, and may occasionally conduct in-person consultations.

Responsibilities

• Conduct telephonic consultations and schedule pharmacist appointments for members enrolled in clinical programs
• Use prescription claims data to generate clinical recommendations (e.g., gaps in care, high-risk medications, adherence, drug interactions, therapeutic substitutions)
• Assess medication regimens for unnecessary or ineffective medications, inappropriate doses, and adverse drug reactions
• Provide patient education on medications and disease states
• Document patient care notes and recommendations clearly and professionally
• Stay current with emerging clinical trends and participate in departmental meetings
• Meet productivity and performance expectations set by management
• Support special projects as needed
