The Lead Primary Care Provider (PCP) at ChenMed plays a crucial role in our mission by :

  • Taking full accountability for patient outcomes while delivering high-quality care to a panel of up to 450 patients.
  • Acting as a health coach, guiding patients in setting and achieving their health goals.
  • Promoting actionable behavior change and facilitating patient engagement to improve health outcomes.

As a value-based organization focused on results, the PCP will work closely with a dedicated care team, participate in ongoing quality improvement initiatives, and champion a culture of positivity and accountability.

Key Responsibilities :

  • Provide care for acute and chronic conditions in older adults.
  • Engage actively with hospital staff when patients require hospitalization.
  • Oversee assessment, diagnosis, treatment, and care coordination for complex health needs.
  • Lead your care team, ensuring comprehensive and compassionate patient engagement.
  • Play an integral role in organizational development and patient recruitment.

    • Qualifications :

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or related specialty required.
  • Current and active State Medical License.
  • Preferred board certification in relevant specialty; board eligibility is required.
  • Current DEA number for controlled substances.
  • BLS certification from the American Heart Association or American Red Cross within 90 days of employment.

    • Compensation :

    $214,700 - $306,714 Salary
