Job Details :

• Board-certified in Internal or Family Medicine
• 4+ years of experience managing Medicare, Medicaid, and commercial populations
• M-F 8AM-5 PM
• Oversee 2 clinics
• Will start out at 50% clinical and 50% admin and then switch over to 70% administrative and 30% clinical
• Ability to treat the acutely ill in an outpatient setting offering hydration, IV, antibioitcs, wound care, and other services
• Chronic illnesses with substance abuse Candidate must be OK with prescribing opioids
• Complex care, Incoming patients are invite-only
• Looking for someone who has experience treating patients with multiple chronic diseases and truly believes in the value of a team-oriented patient-centered medical home model
• EMR is E clinical works, Onsite infusion center
• BLS certification and a strong focus on customer service is required
• Experience in population health management and working with psychosocially and medically complex patients is strongly preferred
