Cultivates a safe, supportive and professional relationship with consumers with a chronic mental illness and / or other disabilities. Develops measurable goals for those consumers, establishing service plans based on consumer needs and desires. Provides skill teaching and assisting with daily living in a residential setting. Documentation of goal progress and relevant personal interventions in an objective, clear and brief manner. Demonstrates a positive cooperative attitude, flexibility, and a willingness to change to meet department goals. Performs additional duties as may be assigned to ensure complete service to all customers.
