Job Description :

✅ Provide skilled PT services to adult and geriatric patients in their homes.

✅ Perform evaluations, develop individualized care plans, and deliver therapeutic interventions.

✅ Accurately document services using the EMR system : Wellsky (Kinnser).

✅ Educate patients and caregivers to improve independence, safety, and quality of life.

✅ Collaborate with nurses and therapists for patient-centered outcomes

Requirements :

✅ Active Texas Physical Therapist License

✅ EMR Experience – Wellsky / Kinnser is a Plus

✅ Home Health Experience is a plus

✅ Current CPR Certification

✅ Reliable Transportation, Valid Driver’s License
