Summary : $5,000 sign-on bonus paid quarterly over first year. We are seeking compassionate and skilled nurses to join our care team model, where you will play a pivotal role in coordinating and delivering person-centered care for people with disabilities. In this role, your time will be primarily focused on directly supporting clients in the community. Your remaining time will be spent performing office-based tasks such as documentation, care planning, medication management and team collaboration.
You will work closely with an interdisciplinary team to ensure individuals with intellectual and developmental disabilities receive the services and support they need to thrive in their daily lives.
Key Responsibilities :
- Client Care Responsibilities :
- Conduct health assessments and develop individualized care plans in collaboration with clients, families, and the care team.
- Respond to acute needs in-person or via phone consultation
- Provide education and support to clients and caregivers on health and wellness topics.
- Monitor and evaluate client progress, updating care plans as needed.
- Advocate for clients to ensure they have access to necessary healthcare and community resources.
- Office-Based Responsibilities :
- Maintain accurate and timely documentation of care activities and client interactions.
- Collaborate with the care team, including case managers, therapists, and other professionals, to develop and review care strategies.
- Participate in team meetings to discuss client progress and address care challenges.
- Stay current on best practices and regulatory requirements in nursing and developmental disability care.

At least 18 years of ageActive RN license in the state of Indiana1 year of experience in ID / DD or long-term care nursing preferredAbility to communicate effectively in order to : Complete required forms and reports of services provided, Follow oral and written instructions, and Communicate with an individual in the individual’s mode of communicationAbility to pass criminal / registry background checks, A valid driver’s license & insured vehicleAbility and willingness to support individuals with challenging behavior and / or medical concernsAbility to pass and maintain required trainings such as CPR / First Aid, Medication Administration and Positive Behavioral Support Training and other trainings as deemed necessary by the agencyAbility to perform light to moderate lifting when neededAbility to train staff, clients and family members on health related topicsWilling to travel primarily within Allen County and occasionally the surrounding counties Easterseals / Arc serves with reimbursement for mileage
