Under the direction of the facility Administrator or designee, the Assistant Case Manager / Transportation Scheduler assists with clinical management of the managed care population of patients, functioning as liaison between facility and managed care / insurance company regarding the scheduling and transportation of medical appointments. DUTIES & RESPONSIBILITIES :
- Functions as liaison between facility and Managed Care / Insurance Company regarding issues related to clinical care provided to Managed Care Company’s patients.
- Schedules the transportation for all residents to / from medically directed and ordered appointments,
- Maintains significant contact with referring health care professionals and associations, nursing service personnel, medical staff, patients / residents, families, insurance representatives, and other staff.
- Coordinates and communicates with all clinical professionals involved with patients / residents.
- Exemplifies a working knowledge of the facility’s Levels of Care provided by the region / market, special programs and designated referral priorities for the region / market.
- Serves as a team member and supports all sales and operational efforts.
- Coordinates and facilitates referring and receiving physician communication.
- May fill in for van driver on weekends on occasion.
QUALIFICATIONS & REQUIREMENTS :
Must have a high school diploma or equivalent; Bachelor's degree preferred.Must have a valid unencumbered driver’s license in the state of operation (no motor vehicle violations including, but not limited to, Driving Under the Influence or other related charges)Prior experience in case management is preferred.Reliable transportation required.
