Location : Tallahassee, FL (Service area including Tallahassee, Crawfordville & Gadsden County)

Contract Duration : 13 weeks (Potential for extension)

Start Date : 12 / 22 / 2025

Schedule : Mon–Fri, 8 : 00 AM–5 : 00 PM (40 hrs / week, on‑call if needed)

Description :

Provide in‑home physical therapy care using HCHB / OASIS. Manage patient visits, treatment plans, documentation, and coordination with care teams.
