Description
The RN Case Manager is responsible for coordinating and managing the care of patients across various healthcare settings, ensuring the implementation of individualized care plans and providing high-quality nursing care. This role requires strong clinical knowledge, excellent communication skills, and the ability to collaborate effectively with an interdisciplinary healthcare team.
Schedule : Full-Time Monday through Friday 8AM-5PM
Primary Responsibilities and Duties :
- Conduct comprehensive patient assessments to identify healthcare needs, develop individualized care plans, and update plans as needed.
- Provide skilled nursing care, including wound care, medication administration, and patient education, in accordance with the care plan and physician orders.
- Coordinate and supervise the activities of healthcare professionals involved in patient care, such as nursing staff and therapists.
- Communicate effectively with patients, families, physicians, and other members of the healthcare team to update them on patient progress, discuss treatment plans, and address any concerns.
- Monitor patients' progress, evaluate the effectiveness of care plans, and make adjustments as necessary.
- Ensure compliance with regulatory guidelines, documentation requirements, and facility policies and procedures.
- Collaborate with an interdisciplinary healthcare team to develop and implement patient care strategies, considering the patient's physical, emotional, and social needs.
- Participate in team meetings to discuss patient care plans, progress, and discharge planning as needed.
- Perform all other duties assigned by the Director of Nursing or other designated supervisor.
Requirements
Job Qualifications :
Valid Registered Nurse License (RN) in the state where work is performed.1 or more years of experience as a case manager or related role in a healthcare setting.Strong assessment and critical thinking skills, with the ability to make sound clinical judgments.Excellent interpersonal and communication skills, with the ability to collaborate effectively with patients, families, and team members.Commitment to providing high-quality, patient-centered care with empathy and compassion.Proficient in basic computer software programs and electronic medical record systems.Knowledge of regulatory requirements and guidelines governing case management and documentation in healthcare settings.Strong organizational and time-management skills, with the ability to handle multiple tasks and prioritize effectively.Physical Requirements :
Ability to lift and transfer patients, as well as carry equipment and supplies weighing up to 50 pounds.Capable of prolonged standing, bending, squatting, and walking during home visits.Fine motor skills for documentation and administering treatments.Ability to respond swiftly and efficiently to emergency situations.Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor / manager.Job requires accepting criticism and dealing calmly and effectively with high-stress situations.The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.Salary Description
$40 - $42 Per Hour
