HonorVet Technologies is seeking a local contract nurse RN Care Manager for a local contract nursing job in Detroit, Michigan.

Job Description & Requirements

Integrated Case Manager – Inpatient (RN or LMSW)

  • Location : Detroit, MI
  • Work Type : Onsite / Contract (10 / 14 / 2025 – 01 / 10 / 2026)

    • Experience

  • Acute inpatient case management : Must have at least 5 years , with recent experience within the last 2 years .
  • Experience in discharge planning and care transitions .
  • Exposure to complex patient populations (multiple comorbidities, high-risk cases).
  • Optional but valuable : home health, rehab, community health, or managed care experience .

    • Epic EMR or other hospital EMR experience .

    Education

  • Nursing degree (BSN preferred if converting to full-time later) OR
  • Master’s in Social Work (MSW) for LMSW candidates.
