Case Manager Float - Full-Time

• Full-time (M-F 8am-4 : 30pm)

Experience in physical medicine and rehabilitation strongly preferred.

This Case Manager will cover the locations in the western St. Louis region - Warrenton Lake St. Louis St. Charles Chesterfield and addition there may be times when coverage of Olivette Kirkwood Arnold and Florissant will be needed.

This position includes mileage reimbursement.

SSM Health Day Institute is seeking a Case Manager to work in our specialized outpatient rehabilitation program and have the ability to float within the region to assist other Day Institute facilities. We have nine centers across the St. Louis Metro area serving individuals recovering from a traumatic injury or illness who no longer require 24-hour nursing or acute rehabilitative care. We take a comprehensive multi-disciplinary approach to build skills and maximize independence. Responsibilities

Under general direction monitors and assesses patient plan of care from pre-admission to post- discharge.

As part of the treatment team assists patient / family in goal setting to maximize patients independence in both home and community. Interacts with payers to obtain necessary authorizations for initiating and continuing care throughout admission to the program.

Qualifications

Professional Healthcare Degree in either

One year medical rehab case management experience (preferred)
