Position Description :

The Life Enhancement Liaison / Assistant works as a liaison between both the clinical nursing team and life enhancement to assist in providing life enhancement education to peers, implement engagement programs on households, provide one-to-one engagement to patients, and promote person-centered care. In collaboration with nursing and life enhancement, the Life Enhancement Liaison will assist in educating PCA's on the household in regards to the benefits of engagement, how to engage various types of patients, and encourage nursing staff to apply their training into small household programs. They will also assist the life enhancement team during community-wide events when needed and work collaboratively to provide a high quality of life and wellness to patients. The Life Enhancement Liaison will provide support and direction to the interdisciplinary team to meet all the needs of the patients, their families and unit staff. They will strive to make every encounter with patients and families into a positive and meaningful experience and opportunity. The Life Enhancement Liaison will also participate in self managed work teams that ensure household activities are consistent with the Hebrew Rehabilitation Center mission, vision and model of care.

Position Requirements :

  • Provide ongoing education to household nursing staff to promote patient engagement and household / neighborhood program opportunities.
  • Act as a liaison between the clinical nursing team and life enhancement to work collaboratively and create 'best practice' policies and procedures for both teams to work more efficiently and effectively together.
  • Implement in-services for departmental, floor, and / or Center- wide staff to educate them about programs which increase understanding about the contribution of life enhancement to community life.
  • Assess and provide the need for one-to-one patient engagement.
  • Work with peers and fellow PCAs to help them learn about and develop engagement opportunities on the household level.
  • Assess patients' capabilities to develop and implement therapeutic programs to meet the physical, cognitive, psychosocial, spiritual and sensory needs of individual residents on their assigned unit.
  • Provide direct patient care and work collaboratively with interdisciplinary team to implement specific patient treatment plans.
  • Document patient progress through notes and other paper work as requested.
  • Assist in coordination of Center-wide activities.
  • Guide volunteers to assist with programs.
  • Participate in process improvement and offer input and ideas to improve systems and processes.
  • Keep patients informed of activities and, working collaboratively with other departments, take responsibility for ensuring transport of residents to and from programs as needed.
  • Assist in departmental duties that keep the department neat, orderly, and professional.
  • Increase the knowledge base through participating successfully in classes, conferences, and workshops both within and outside of the Center. Apply knowledge gained.
  • Provide additional training and support to staff that needs help with documentation; audit documentation on assigned floors as requested.
  • Assist Nurse Managers with floor projects and other unit-specific needs.
  • Meet with supervisor regularly to review the Community Life on the assigned units, address problem areas (including clinical issues), and determine the progress toward the individualized annual goals.
  • Perform other related duties as requested.
  • Qualifications :

  • Current Certified Nursing Assistant (CNA) certificate in good standing or current enrollment in a nursing school, having completed first clinical experience.
  • Experience leading groups programs with patients in an LTCH
  • Excellent communication skills.
  • Computer literacy required; experience with databases, Windows, Word, Excel, and databases.
  • Must be professional, proactive, collaborative, conscientious and results-oriented individual.
  • An advocate and leader for the vibrancy and engagement of the community.
  • Flexible to meet the needs of the overall operation of HRC.
  • Physical Requirements :

  • Push occupied wheelchairs on flat surfaces and up / down ramps up to 50 lbs.
  • Standing, walking, and sitting.
  • Transporting and assisting with transferring patients.
  • Lifting supplies and equipment up to 50 lbs.
  • Some reaching, stooping, squatting, bending, kneeling, and crouching

    Salary Range :

    $52,913.00 - $79,370.00
