Family First Healthcare Services -

Family First Healthcare Services in Florida is seeking a Clinical Liaison in home healthcare (Bi-Lingual in English and Spanish preferred) who is highly capable of building relationships to coordinate patient care transitions between different facilities and our agency. Key responsibilities include assessing and screening potential patients, coordinating admissions and transfers, and serving as a communication bridge between referring providers, patients, families, and our agency's clinical team.

As a family of caregivers, our patient centric approach and enhanced emphasis on optimal patient outcomes is deeply rooted in every one of our interactions. This Clinical Liaison is expected to be fully dedicated to the wellbeing of our patients, partners, referral sources, and clients. Our Home Care team leads by example—ensuring that the promise of our culture of care is delivered to each and every individual our organization has a relationship with, from clients, patients to everyone in between.

Our Clinical Liaison will work in close concert with of our Head of Business Development & Growth. This person will also act as a marketer for our organization and provide education to patients and families.

Key responsibilities

  • Patient assessment and coordination :  Evaluate potential patients by reviewing medical records and conducting pre-screening evaluations to determine eligibility for admission.
  • Admissions and transfers :  Facilitate patient admissions and coordinate transfers to our agency from acute care, rehabilitation or skilled nursing facilities, Physician practices, Payors, and other appropriate sources.

Relationship building :  Cultivate and maintain relationships with external referral sources, including hospitals, physicians, case managers, discharge teams, and Payors to increase patient referrals.

Patient and family education :  Educate patients and their families about our agency's services and what to expect during their stay, providing comfort and support. Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of post-acute care.

Ability to review patient medical charts and understand test results, therapy evaluations, preexisting conditions, and have a general medical knowledge of the patient. Ability to clearly and professionally interact with patients, families, and healthcare providers while gathering additional clinical information and past history

Experience conducting thorough patient assessments to identify patients for potential admission into the rehabilitation program. Able to complete detailed Pre-Admission Screens, as applicable, according to agency's policies and procedures. Schedules meetings and arranges services for medical professionals including potential and existing referral sources, doctors, nurses, social workers and other health care professional.

Provide patient updates to physicians, payers, case managers, social workers and other relevant persons. Collaborate with caregivers and office staff to ensure seamless coordination of services.

Liaison and communication :  Act as the primary point of contact, ensuring clear and consistent communication between the patient, their family, the referring sources, and Family First’s receiving clinical team. Conduct in-home or facility assessments to determine client needs and develop individualized care plans. Maintain solid working relationships with new and existing referral sources by providing excellent after-sales service.

Marketing and promotion :  Promote the organization's services to potential referral sources and qualified patients and their families, sometimes by leading tours or giving presentations. Educate referral sources and community partners Family First’s available home healthcare and home care services options.

  • Documentation :  Complete required paperwork and maintain accurate records related to patient referrals, progress, and payer information.
  • Assists patients in the process of navigating post-acute care with an overall goal of creating a positive impact on patient outcomes and referral source satisfaction. Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of post-acute care.
  • Able to travel within assigned market. A self-starter who possesses a profound sense of ownership and dedication to clients’ wellbeing and success. Ability to work independently with minimal supervision.
  • Ability to achieve and surpass established productivity metrics and enable the agency’s overarching mission and goals. Assist with follow up with clients and families to ensure satisfaction and adjust care plans as needed.

    Actively participate in outreach events and community networking to strengthen referral relationships. The Clinical Liaison serves as the primary connection between referral sources, clients, families, and our home care team. This role focuses on client intake, coordination of care plans, communication between healthcare partners, and ensuring high-quality, client-centered service delivery.

  • Conduct in-home or facility assessments to determine client needs and develop individualized care plans. Collaborate with caregivers and office staff to ensure seamless coordination of services. Educate referral sources and community partners about available home care options. Participate in outreach events and community networking to strengthen referral relationships.
  • Required Qualifications and Skills

  • Strong clinical background, preferably an RN, LPN, or similar license.
  • Excellent communication, interpersonal, relationship building, and negotiation skills.
  • Knowledge of healthcare systems, case management, and patient advocacy.
  • Strong organizational and time-management abilities.
  • Proficiency with computer skills and electronic health records.
  • A Minimum LPN / LVN or Registered Nurse license or minimum similar healthcare credentials such as an Associates or Bachelors degree in healthcare related field.
  • A minimum of two years of relevant direct home healthcare industry specific work experience.
  • Compassionate, focused, patient centric attributes.
