Are you open to considering a stronger opportunity? We are seeking a physician for a Priority Care Program. In this role, you will join an innovative care delivery team located in the San Fernando Valley.

Our client, an established and growing provider organization, needs an experienced physician to provide post-discharge clinical care for a select patient population.

Collaborating with designated PCPs and Nurse Practitioners, the Priority Care Physician will ensure high-quality, transitional care for more complex patients.

DETAILS :

• Provides care in an outpatient clinic / community environment
• Facilitates interdisciplinary meetings to ensure coordination of care
• Supported by a collaborative Care Coordination team
• Anticipate a minimum of 4 days per week in the clinic setting
