$4000.00 Sign-On Bonus-Valid from 12 / 3 / 2025-12 / 31 / 2025

The bonus will be paid in two installments :

• $2000.00 after 6 months of continuous employment
• $2000.00 after one year of continuous employment

Coverage Area : Northern Lawrence

FLSA Status : Exempt, Full-Time

Salary : $82,000-$89,000

Schedule : Monday-Friday, with occasional on-call requirements

Position Summary

The Hospice RN Case Manager provides compassionate, patient-centered nursing care focused on supporting comfort, dignity, and quality of life for individuals receiving end-of-life services in the home setting. This role manages an assigned caseload and ensures each patient's individualized hospice plan of care is developed, implemented, and evaluated according to VNA standards, clinical best practices, and state and federal regulations.
