Schedule

  • 5 eight hour shifts, weekdays, occasional back up on-call rotation Full-time (40 hours)
  • Serving Trellis Supportive Care Forsyth county and bordering counties - office located in Winston Salem, NC

    • Position Summary :

    The Hospice Homecare Nurse makes visits and provides hospice care in patients homes and facilities and plans, organizes, and directs the hospice home care services to support patients living with a life-limiting illness. The hospice nurse is responsible for direct patient care, making visits in homes and facilities, by assessing needs and developing and implementing an individualized plan of care that maintains the highest level of comfort and quality of life. The Hospice Nurse coordinates and supervises the care using an interdisciplinary team approach. In addition, the Hospice Nurse performs ongoing assessment and revises the initial written plan of care with team collaboration as the needs and conditions of the patient / family change.

    Position Education / Skills / Abilities :

  • Excellent nursing assessment and problem-solving skills
  • Excellent interpersonal communication skills, both oral and written
  • Exceptional customer service required
  • Sensitive to the needs of terminally ill patients and families and one’s own feelings about death and dying
  • Tolerant of individual lifestyles
  • Computer proficiency with electronic medical records
