Experience : Two or more years healthcare experience, preferred. Case Management or related experience, preferred. Licensure / Certification : Current RN licensure in state of employment, required. Additional skills required : Weekend and holiday coverage. Additional skills preferred : Knowledge / Competence of Microsoft office products and keyboarding. Complex discharge planning Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive / complex / safe discharge plans Collaborate with MSW case managers for guidance with pscyhosocially complex cases Facilitate coordination of care Accept responsibility, authority, and accountability for management of care Serve as a resource for discharge planning and care coordination for medically complex cases Maintain current knowledge of available community resources / post acute care options
What We Offer :
Schedule : PRN; Shifts may be available any day of the week between the hours of 8 : 00 AM and 5 : 00 PM; must be able to train during the week for several weeks
Location : Presbyterian Medical Center - Charlotte, NC; Presbyterian is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte; will be eligible to float to other hospitals in the region
This role is ideal for an experienced acute care RN looking to work in a patient facing but non-bedside role within a high-performing, collaborative team.
