We are seeking an experienced and compassionate Program Director to lead a well-established Adult Day Care program dedicated to delivering high-quality, person-centered services grounded in the social model of care. This leadership role focuses on fostering a supportive and engaging environment that promotes social interaction, well-being, and independence for program participants.

Develop, implement, and oversee programs that encourage social engagement and community participation.

Monitor program outcomes and apply data-driven improvements.

Establish and maintain policies aligned with the social model of care.

Conduct staff evaluations and coordinate ongoing training initiatives.

Cultivate partnerships with community organizations, healthcare providers, and families.

Manage budgets and allocate resources to maintain program sustainability.

Identify and pursue funding opportunities, including grants.

Analyze and report on program data to guide strategic planning.

Advocate for participants’ needs within the broader community and healthcare systems.

Stay informed of industry best practices and trends related to adult day care services.

Bachelor’s degree in a healthcare or related field preferred.

Experience managing programs in adult day care or social services preferred.

Strong knowledge of the social model of care and relevant regulatory requirements.

Proficiency in data management and reporting tools.
