Physical Therapist - Home Health - Full Time - York / Fillmore / Saline Counties, NE

Tabitha's Home Health in York / Fillmore / Saline Counties, NE is seeking a Physical Therapist to work full time 8a- 4 : 30p.

The Physical Therapist is responsible for providing Physical Therapy services to Tabitha's Home Health Care Seniors / residents / clients with physical or cognitive disabilities to maximize functional independence.

Essential Functions

• Evaluates clients utilizing appropriate diagnostic muscle, nerve, joint and functional ability tests.
• Performs Physical Therapy treatment procedures or delegates to a licensed Physical Therapist Assistant based on initial and on-going client evaluation.
• Develops and implements the client's plan of care in collaboration with the physician, other consulting health care professionals, family / caregiver according to agency procedures and state and federal regulations.
• Documents the care provided, instructions given, client's progress toward goals, and changes in condition adhering to Tabitha's policies and procedures and state / federal regulations.
• Reports significant changes in the client's condition to the physician and nursing staff.
• Communicates and coordinates care and client's progress with other team members involved in the care.
• Designs, implements, and evaluates therapeutic exercise programs, and instructs the client's family / caregiver / Home Health Aide in carrying out the program.
• Consults with the attending physician and obtains a written or telephone order for Physical Therapy plan of care and any changes in the client's plan of care as needed.
• Ensures that assigned client's clinical records are complete, timely, and reflect coordination of care.
• Oversees and coordinates Physical Therapist Assistant and Rehabilitation Aide client care activities.
