Assures adherence to agency policies and procedures. Applies the nursing process competently in care delivery. Responsible to the Clinical Manager. Supervises and evaluates Home Health Aides and Licensed Practical Nurses. Must travel for home visits and periodic office meetings. Reliable transportation, current driver's license, and agency-required liability insurance are necessary. Average number of patients per day is 5-8. Various types of visits and activities carry different point values. Contractor will be assigned between 5-10 patients per day. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Must be willing to travel up to 75 miles from home. Mileage is reimbursed at the IRS standard rate. All documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation will impact billing and follow-up patient care. Candidates must accept every patient on their schedule regardless of distance. Position is for a vacant opportunity with potential for full-time employment. Candidates must have experience in home health or hospice care. On-call requirements vary; weekend coverage is every third weekend. Clinic operates Monday through Friday, 8 AM to 5 PM. Must have recent experience with HomeCare HomeBase. Proficiency in OASIS is required; retraining may be necessary if not used in the last six months.
