Position Summary

The Occupational Therapist (OT) provides medically prescribed, goal-oriented therapy in accordance with the Physician's Plan of Care , company policies, and professional standards of practice.

You will collaborate with an interdisciplinary team to coordinate care and ensure the highest level of patient outcomes.

Key Responsibilities

• Develop and implement individualized patient occupational therapy plans of care
• Conduct in-home evaluations and assessments
• Communicate with physicians, nurses, and care team members as needed
• Provide education and training to patients, families, and caregivers
• Participate in interdisciplinary team meetings to coordinate and review care plans
• Ensure timely, accurate, and compliant electronic documentation
• Apply agency policies and adhere to all state and federal regulatory standards
• Continuously evaluate patients' progress and update care plans accordingly
