job_description.job_card.job_descriptionPerform Physical Therapy assessments and develop treatment programs Record and report the patient's reaction to treatment and condition changes Instruct patients and caregivers in therapy programs and device use Supervise physical therapy assistant and, when appropriate, the certified nursing assistant Prepare reports and participate in staff and Agency development activities Monday Through Friday 8 : 00a / 9 : 00a-5 : 00p plus 1 weekend per month (Saturday & Sunday; day shift with scheduled patients) 2 holidays (1 summer / 1 winter) No on-call Local travel to residential homes
Qualifications : Required :
- Master's degree or higher
- Over 1 year of Healthcare Setting experience
- Valid Physical Therapist in the State of Illinois (IL)
- Valid Regular Driver License (DL)
Overview :
You will be responsible for providing Physical Therapy services to patients in their homes, assessing patient functioning levels, and developing treatment programs to improve independence and physical / cognitive functioning. You will also be involved in patient and caregiver education, supervision, and reporting on patient progress.
