Rate Caps for Submission : Malpractice Must be included in rates.Orientation 210 - half the hourly / callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours On Call Daily 3100 - Per 24 hours to include 4 hours gratis24 Hour Holiday Call Rate - 4650 - Per 24 hours to include 4 hours gratisCall Back 420 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms. Holiday Call Back 630 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms. Schedule / Availability Requirements : Dec 15-17 aPotentially 2 weekends per month ongoing pending PAC approvals for the additional coverage. Details : 24 hour call 7a-7a Setting : Inpatient Call Trauma Level 1Required Procedures / Job Duties : Standard ENT coverage.EMR : EPICSupport Staff : NoneReason for Coverage : Provider resigned leaving gaps in call coverage.EMR Required Training : Credentialing Preapp and full app must be completed and submitted to move provider into applicant in training to generate AD account and begin EMR account set up. Once AD account creation begins it can take up to 5 days to create account. Module learn lessons Must be completed prior to virtual EMR training to attend the training.EMR Virtual PPL training Only able to attend if Modules are completed before training. Virtual training must be completed at least one week prior to the providers first day. First Day Details - Site requires completion of EMR Modules and training before first day details can be released. All candidates must complete the following EMR questionnaire and detail it in the notes section of the candidates submission at time of presentation.What are the # of years of epic experience they have?When was the last time MM / YYYY that they used Epic?What is the name of the last facility the provider used EPIC?Can you provide me with a photo large enough to add to a badge and not become pixelated? - Please attach photo in documents sectionCan you disclose the provider's time zone for scheduling the virtual epic training?We need the provider's best phone # to use for call shifts while working.
