Registered Nurse - Home Health

Job Summary

We are seeking a dedicated and compassionate Registered Nurse (RN) to provide in-home healthcare services to patients. The ideal candidate will deliver high-quality nursing care, ensure patient safety, and support recovery in the comfort of the patient's home. This role requires strong clinical skills, autonomy, and experience with OASIS documentation.

Key Responsibilities

• Deliver patient-centered nursing care in home health settings.
• Complete comprehensive assessments using OASIS documentation standards.
• Develop, implement, and evaluate individualized care plans.
• Collaborate with interdisciplinary teams to coordinate patient care.
• Monitor and document patient progress using EPIC electronic health record system.
• Educate patients and families on treatment plans, medications, and health maintenance.
• Maintain accurate and timely documentation in accordance with regulatory requirements.
