Job Summary :

As a Physical Therapist, you will evaluate patients' functional levels and create tailored care plans under the guidance of an M.D. You'll coordinate care with other agency personnel and community resources, oversee the work of Physical Therapy Assistants (PTAs) and Home Health Aides, and ensure accurate clinical documentation.

Key Responsibilities :

• Manage a daily patient caseload.
• Evaluate patients and develop comprehensive care plans in collaboration with M.D.s and the Hospice Interdisciplinary Team.
• Implement safe and efficient care plans, including for hospice patients, focusing on patient safety and education for caregivers.
• Regularly assess care plans and adjust as needed based on patient needs.
• Collaborate with a multidisciplinary team, including nurses, social workers, and physicians.
• Supervise PTAs, delegating appropriate care responsibilities.
• Maintain organized and prioritized caseloads, ensuring all patient needs are met effectively.
• Ensure compliance with clinical documentation standards and agency policies.
• Prepare accurate discharge records and communicate effectively within the agency.
