A little more about our center :

West Chicago Terrace is an Intermediate Care Facility (ICF) with 120 beds, proudly serving the behavioral / psych population. We are in a quiet residential area, across the street from an elementary school and easy to commute to. A few key highlights :

• Highly ranked Quality Measures - this means we provide excellent clinical care!
• The average age of our patients is 35-55 and the most common diagnoses are schizophrenia, bi-polar and major depressive disorders.
• Our residents have created a garden club, a poetry club and are eager to interact with you. Many of them return to the community (or home) after residing with us.
• We welcome new grads and work with students from multiple schools / universities in the area.

Our nurse orientation program is extensive - let us teach you.

Job Description

What you will do in this role : As a Registered Nurse (RN) or Licensed Practical Nurse (LPN), you will be joining a team of nursing professionals dedicated to providing excellent clinical care. Some have been with us for a long time and can mentor others. You will help patients reach their clinical goals and work side by side with a highly skilled multidisciplinary team.

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
