Job Overview – Nurse Manager (Home Health Care) :

Compensation : $140,000 – $150,000 / year

Location : Stamford, CT

Schedule : Monday to Friday, 9 : 00 AM – 5 : 00 PM (In-Office with Local Travel)

Our client is seeking a Nurse Manager (Home Health Care) in Stamford, CT, to lead clinical operations, nursing staff management, and patient care. You will oversee field nurses, manage admissions and care planning, ensure compliance with OASIS and CMS regulations, and conduct quality reviews. Ideal candidates are experienced RNs with leadership skills and expertise in home health standards.

Responsibilities as the Nurse Manager (Home Health Care) :

• Clinical Supervision : Lead, mentor, and evaluate field nurses and clinical staff through education, performance reviews, and ongoing support.
• Care Coordination : Oversee patient admissions, care planning, and case management to ensure timely, high-quality care.
• Regulatory Compliance : Ensure accurate and timely OASIS documentation, Plans of Care, and 485s in alignment with CMS and state requirements.
• Quality Assurance : Conduct chart reviews and home visits to maintain clinical excellence and compliance standards.
• Collaboration : Partner with physicians, caregivers, and families to develop and adjust individualized care plans.
• Staffing & Recruitment : Support hiring, onboarding, and retention of qualified nursing staff to strengthen the home health team.
