Knowledge of advanced directive and ability to assist patients and families with completion when needed
Able to work independently and as part of a multidisciplinary team
Knowledge and skills to screen patients for palliative care needs
Ensure effective communication is established with the patients, caregivers, families, and all disciplines to facilitate a successful transition across the continuum of care
The ability to ensure the confidentiality and rights of all patient and departmental documents is required
The ability to effectively communicate and teach stages of chronic conditions
The ability to effectively communicate end of life care and make referrals to Hospice care when needed
Qualifications
To qualify for the position, the candidate must have a current licensure as a registered nurse. The candidate must have at least two years’ experience in a hospital setting. The candidate is preferred to have a BSN or other related bachelor level degree. Certification in palliative care not mandatory to start, yet requested to be completed within 2 years of employment. An authentic belief and acceptance of Palliative Care principles is essential.
