This position requires commitment for 22.5 hours a week, offering flexibility for either three full days or five shorter days.
As a Participant Referral Specialist, you will be the welcoming first point of contact for our participants. Your ability to compassionately guide them through our triage and assessment process is crucial in understanding their needs, preferences, and goals. Your main responsibilities will include :
- Receiving and processing service referrals through various channels (email, text, referral form, web-form, and telephone).
- Providing exceptional customer service with a focus on compassionate communication and effective triage.
- Facilitating seamless onward referrals and sign-posting to ensure a positive experience for participants.
- Managing the referral inbox and supporting the onboarding of participants onto our virtual care platform.
- Collecting necessary participant data through questionnaires and ensuring their needs are met throughout their journey.
- Conducting proactive case management and responsive engagement with participants using various communication methods.
- Maintaining compliance with business and contract performance requirements, including data protection and health and safety standards.
Are You the Right Fit?
We are seeking individuals with the following qualifications :
A proven track record in customer-facing roles, with experience in caseload management.Ability to support vulnerable individuals during changes.Proficient in using IT systems, specifically Microsoft Office.Experience of data inputting and managing customer needs while maintaining composure under pressure.Background in health, community, or social care environments is advantageous.Key Competencies :
Understanding social determinants of health and behaviour change principles.Excellent organizational skills and attention to detail.A non-judgmental approach with a genuine desire to improve the lives of others.Flexibility to work evenings and weekends as required, ensuring accessibility for all participants.What We Offer :
