Children and Pregnant Women Case Manager

MyCHN is a nonprofit organization providing Medical, Dental, Behavioral Health, and Pharmacy services to communities in the South Houston and Gulf Coast area. We were founded in 2008 with one provider, three exam rooms and a big dream. Today, we have over 100 providers and more than 15 locations. The Children and Pregnant Women (CPW) Case Manager supports the care coordination program and MyCHN clinics through case management of pregnant women and children. This position involves rendering care through Texas Medicaid CPW services at MyCHN. The CPW Case Manager will help patients access needed medical, social, educational and other services, aligning with policy guidelines to ensure compliance.

Duties and Responsibilities :

• Collaborates and coordinates with the providers of direct services to ensure the patient's care plan is followed and to avoid duplication of services.
• Coordinates with managed care coordinators.
• Provides timely documentation on electronic health record on each service offered.
• Provides all aspects of case management services to pregnant women and children, from initial assessment to follow-up.
• Reviews every aspect of the patient's and family's life for factors that contribute to healthy outcomes.
• Collaborates with the patient or family, the referring provider, and service providers to make sure the patient or family receives the help they need.
• Participates in community events and networks with community agencies who serve pregnant women and children.
• Comply with all licensure requirements of the appropriate state licensure / examining board, including the obligation to report all suspected child abuse / neglect
• if the case manager's professional license is suspended or revoked, cease providing services and notify the department with such notification to be provided to the department no later than seven calendar days after the date that the suspension or revocation is imposed
• Provide services convenient to clients, either in their home, an office setting, or other place of client's preference
• Have knowledge of, and coordinate services with, providers of health and health-related services and other active community resources.
