Position Overview
The Registered Nurse (RN) – Home Care Administrator is responsible for the day-to-day administrative and clinical coordination of Seva Home Care’s operations. This role blends RN-level oversight with administrative leadership , ensuring compliance, efficiency, and high-quality care delivery under Georgia DCH and Medicaid waiver program standards.
While the RN will occasionally perform direct nursing duties such as assessments or supervisory visits, their primary focus is on coordinating care, overseeing service delivery, maintaining compliance, and supporting the operational workflow of the PSS department. This individual will also collaborate with other Seva departments (ADH and SFC) as needed, though such cross-departmental support will be infrequent and situational .
Key Responsibilities
Administrative Leadership & Coordination
- Oversee and coordinate all service starts for new home care clients, ensuring timely onboarding and complete documentation.
- Manage scheduling reviews for caregivers and clients to ensure coverage, continuity, and accuracy.
- Track and coordinate RN visit assignments (Initial Assessments, Reassessments, and Supervisory Visits) with supporting nurses.
- Assist with timesheet sign-offs and ensure submission compliance within DCH timelines (not before 14 days, within 30 days).
- Conduct chart audits and maintain up-to-date client records in both Caryfy (EHR) and physical binders.
- Communicate with case managers and referral sources to resolve service or compliance issues.
- Support the intake process by reviewing new referrals for eligibility, documentation, and readiness for service start.
- Generate, organize, and submit reports for compliance, visit tracking, and active client census.
Clinical Oversight & Quality Assurance
Coordinate the completion and timely submission of all Initial Assessments (IAs) , Reassessments (RAs) , and Supervisory Visits (SVs) in accordance with Medicaid timelines.Review clinical documentation for completeness, accuracy, and compliance before final sign-off.Step in to perform or co-sign IAs, RAs, or SVs as needed to support the nursing team ormeet compliance deadlines.
Monitor health-related changes in clients and ensure appropriate updates to care plans.Collaborate with field nurses and caregivers to ensure care delivery aligns with clientneeds and approved service hours.
Provide coaching, feedback, and guidance to caregivers and administrative staff oncompliance and care documentation.
Compliance & Documentation
Maintain compliance with DCH, HFRD, and Medicaid waiver program standards for home care.Ensure all required forms and documentation (care plans, timesheets, assessments, supervisory notes, etc.) are accurately filed.Monitor and ensure each client’s EDWP packet and Seva chart remain audit-ready.Support preparation for audits, recredentialing, or licensing renewals .Assist HR with maintaining caregiver compliance items (TB, CPR, background checks,etc.).
Communication & Team Collaboration
Serve as the central point of coordination between nursing staff, caregivers, intake, HR, billing, and leadership.Maintain professional communication with clients, families, case managers, and external partners.Participate in weekly departmental meetings and compliance reviews.Step in to assist ADH or SFC departments as needed for short-term support orcross-training initiatives.
Continuous Improvement
Identify workflow bottlenecks and recommend improvements to enhance departmental efficiency.Support technology adoption and digital systems (Caryfy, Monday.com , SFAX, RingCentral, Google Workspace).Participate in Seva-wide training and process development efforts.Qualifications
Active Georgia RN License (Required)CPR / BLS Certification (Required)Minimum 2 years of experience in home care, personal support, or community-basedhealth services
Knowledge of Medicaid waiver programs (CCSP, SOURCE, SFC) preferredStrong administrative and organizational skillsExperience using EHR platforms (Caryfy, Alora, or equivalent)Proficiency with Google Workspace, GAMMIS, and Monday.comReliable transportation, valid driver’s license, and proof of insurancePerformance Indicators
