Description :
Establishes and maintains procedures to identify sponsors whose family members have special medical and / or educational needs in a timely manner.
Conducts assessments of family members of Regular Air Force (RegAF) sponsors to determine if special medical and / or education conditions exist that require Exceptional Family Member Program (EFMP) enrollment.
The SNC is involved in the Family Medicine Relocation Clearance (FMRC) process as outlined in AFI 40-701, Medical Support to Family Member Relocation and Exceptional Family Member Program.
Hours of Performance :
Full-Time Position
40 hours per week
Monday - Friday 0730 - 1630
Weekends - No
On-Call - No

QUALIFICATIONS :
Education Masters Degree in Social Work
Current Medical Social Worker License.
Basic Life Support Certification
Experience 2 years of full-time experience working in care coordination, discharge planning, or case management in a medical setting
