About the Role :

As a Home Health Manager , you'll lead a high-performing team delivering exceptional care to participants in their homes and within nursing or board and care facilities. This role is pivotal in ensuring seamless, personalized, and high-quality care for a frail and elderly population.

Key Responsibilities :

Direct daily operations of the home care department, ensuring all workflows and policies are followed.

Hire, train, mentor, and evaluate home care team members to ensure optimal performance.

Oversee nursing assessments and interdisciplinary team (IDT) participation to evaluate care effectiveness.

Ensure documentation and delivery of nursing interventions align with participants' care plans.

Supervise the quality of care delivered in external nursing and board and care homes.

What Youll Bring :

Bachelors Degree preferred (or equivalent experience).

At least 2 years of nursing experience in home health, preferably with frail or elderly populations.

Valid California RN License .

Deep understanding of nursing care for medically complex and elderly patients.
