We are currently seeking a Social Worker.

Job Responsibilities and Duties :

Plan, develop, and manage social services programs within the facility.

Assist residents with admissions, Medicaid processes, and MDS assessments.

Provide emotional support and counseling to residents and families.

Coordinate community resources and resident appointments as needed.

Maintain compliance with state regulations and professional standards.

Requirements :

Licensure / Certification :

Education :

Minimum of a Bachelor’s degree in social work; OR

Minimum of a Bachelor’s degree in a human-services related major, including sociology, gerontology, psychology, rehabilitation counseling or special education; AND

Social work degree or equivalent
