NURSE PRACTITIONER

Salary Pay Range : $100,00-$115,000

Salary Based on Experience

The Nurse Practitioner (NP) provides comprehensive medical care to people with intellectual and developmental disabilities (IDD), including assessments, diagnosis, treatment planning, medication management, health education, and coordination with other healthcare providers and support systems, all while considering the unique needs of this patient population. The primary role of the NP is to provide in-home medical appointments at People Inc residential homes to avoid unnecessary Emergency Department and urgent care visits.

ESSENTIAL FUNCTIONS / RESPONSIBILITIES :

• Patient assessment and diagnosis : Conducts thorough physical examinations, review medical history, and assesses the person's specific needs related to their IDD, including cognitive, behavioral and communication challenges. Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures needed. Orders / performs appropriate laboratory and diagnostic procedures.
• Care Plan / Plan of Nursing Services (PONS) development : Creates personalized care plans based on assessments, including preventative care, medication management, and treatment strategies tailored to the individual's needs.
• Medication Management : Prescribes and monitor medications, considering potential interactions and side effects specific to individuals with IDD.
• Health education and counseling : Provides health education to individuals with IDD and their caregivers regarding conditions, medication adherence, healthy lifestyle choices, and self-care practices.
• Collaboration with other healthcare providers : Works closely with primary care providers, specialists, therapists, social workers, and other healthcare professionals involved in the individual's care to ensure coordinated care.
• Advocacy : Advocates for the needs of individuals with IDD, including access to necessary healthcare services and community.
• Maintains CPR certification and performs CPR when necessary.
• Complies with all agency policies and procedures.
• Other duties as assigned by Nursing Administration
