Practice Transformation Coach PACE

Position Summary

This position will serve as the practice transformation coach and will provide expert guidance and support to participating pharmacy sites in implementing innovative care models aligned with the goals of the grant. This position will play a key role in preparing pharmacies for service implementation assisting with the successful launch of new services and fostering continuous improvement through ongoing engagement. The coach will conduct regular check-in calls to assess progress identify challenges and provide tailored recommendations to ensure effective and sustainable practice transformation.

Required Qualifications Competencies And Experience

Required Qualifications :

• Graduated from an accredited PharmD program.
• Strong foundational knowledge of pharmacotherapy patient assessment and pharmacy practice.
• Effective communication and interpersonal skills.
• Ability to work collaboratively with faculty and students.
• Must be licensed to practice pharmacy in North Carolina.
