Travel Licensed Practical Nurse (LPN) - Northern Illinois

Connected Health Care - LTC / Rehab (Neuro Rehabilitation)

Up to $1,700 / week | 13-Week Contract | Day or Night Shift | 3x12s | 36 Hours

Connected Health Care, a premier recruitment and staffing firm, is seeking compassionate and dedicated Travel Licensed Practical Nurses (LPNs) to support our partnered long-term care and rehabilitation facility in Northern Illinois . This not-for-profit rehabilitation center specializes in care for young adults with traumatic brain injury, spinal cord injuries, and other neurological disorders .

This assignment is ideal for LPNs who enjoy building meaningful patient relationships while gaining hands-on experience in a specialized neuro-rehab setting.

Position Details

  • Contract Length : 13 weeks
  • Schedule : Day or Night Shift
  • Hours : 3x12s | 36 hours / week
  • Setting : LTC / Rehab - Neuro & Complex Care
  • Compensation : Up to $1,700 per week
  • Start Date : ASAP or upon credentialing

Job Responsibilities

  • Deliver compassionate, high-quality nursing care to patients recovering from neurological injuries and conditions.
  • Monitor patient status and report changes to the RN and provider team.
  • Administer medications and treatments per physician orders and facility protocols.
  • Assist with activities of daily living (ADLs) and rehabilitation support.
  • Collaborate with interdisciplinary rehab teams including nursing, PT, OT, SLP, respiratory therapy, and behavioral health.
  • Accurately document patient care and progress.
  • Maintain compliance with all facility policies and state and federal regulations.

    • Education & Certification Requirements

  • Active Licensed Practical Nurse (LPN / LVN) license - Illinois or compact state.
  • Minimum 1 year of LPN experience - rehabilitation, neuro, TBI / SCI, or LTC experience preferred.
  • Current BLS certification (AHA).
  • Strong communication, teamwork, and documentation skills.
