As a Weekend On-Call Hospice RN, you will provide essential support to patients and their families during evenings, nights, weekends, and holidays, ensuring compassionate care when it matters the most. Enjoy a full-time Baylor schedule with comprehensive benefits and paid time off.

Your Role as a Weekend On-Call Hospice RN :

• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior.
• Deliver direct patient care and support after standard business hours.
• Act as the main point of contact between patients, families, physicians, and the hospice team during your shift.
• Respond promptly and with compassion to urgent calls and patient visits for symptom management.
• Evaluate both patient and family needs to guide clinical decisions and implement appropriate care plans.
• Collaborate with an interdisciplinary team, including LPNs / LVNs, CNAs, and other supporting staff.
• Accurately document all clinical visits and communications.
• Participate in quality assessment and performance improvement initiatives.
• Work independently while ensuring seamless coordination with the daytime care team.
