job_description.job_card.job_descriptionCore Job Responsibilities : Attend meetings focused on managing patient care to find ways to improve the treatment provided and suggest solutions when issues arise. Help organize monthly training sessions for non-medical staff about important topics related to social work and how it connects to patient care. Refer patients to appropriate services and resources. Work closely with case managers to create and carry out a program aimed at assisting patients effectively. Develop local strategies that bring together the skills of both nursing and social work to enhance patient care. Assist in creating and implementing educational programs. As a social work case manager, you might also provide additional services that require a licensed clinical social worker. Make sure patients are connected to community resources and other government services that can help them. Evaluate the emotional and social needs of patients and their families and work with them to create a treatment plan that addresses their issues and strengths, along with the support they need. Conduct assessments and provide support to a diverse group of individuals, considering their various backgrounds, including social and economic factors. Use knowledge of medical and mental health conditions, treatments, and relevant terminology to assist patients. Work Schedule :
Monday to Friday, working 9 hours a day between 7 : 00 AM and 5 : 00 PM, with a one-hour lunch break.
- Minimum Qualifications :
- Education :
- A Master’s degree in Social Work (MSW) from an accredited program.
- Experience :
- At least one year of experience in clinical social work or healthcare management within the last five years, working with adults, children, families, seniors, or groups.
- Licensing :
- Must have an active license as a Licensed Clinical Social Worker (LCSW).
- Certification :
- Current Basic Life Support (BLS) certification from either the American Red Cross (ARC) or the American Heart Association (AHA).
- Security Clearance :
- Ability to pass a government background check.
